

Phase I (First 6 Months) - Clustered Living
Phase II (6-12 Months) - Transitional Living
Phase III (6 Months) - Community Living
Phase IV (3 Months) - Aftercare
During this phase, which is called clustered community living, the men live together in shared supervised living quarters. A Case Manager works closely with them onsite, as well as a Peer Support Specialist. While in Phase I, the residents are expected to:
Seek employment
Begin budgeting program
Obtain a General Equivalency Diploma (GED) or acquire a new skill through vocational rehabilitation or job training
Participate in mental health counseling, substance abuse treatment sessions, and/or parenting classes
Attend Bible study voluntarily
Fulfill random urine screens and curfew checks
Begin weekend visits with family and/or friends
During this phase, the men work on reuniting with family members
Men live in a clustered community, but in their own unit under supervision.
The men receive help filing for custody arrangements of children and assistance with any type of visits that are granted by the Court.
The men are reunited with their children and/or significant other and are participating in community activities.
A housing analysis is conducted to determine the best type of permanent housing for independent living.
Monthly housing inspections are conducted during this phase with random urine testing and Case Management continuing.
Create a budget and develop a savings plan.
Work on building credit.
During this phase, the men are more independent but still receiving support from staff:
The men have secured permanent affordable housing.
The men are living independently in permanent housing units, fully employed.
The men are still connected to the support system, but with decreasing Case Management.
The men are now fully employed, learning to navigate within peer networks and in the community.
During this phase the men are now in permanent housing. They are monitored in all areas of previous phases. The men in this phase should be managing areas such as:
Finances (paying rent on their own and on time, maintain a checking and savings account)
Parenting
Staying clean and sober
Maintaining employment
Maintaining healthy relationships with family members and friends
