The Case Manager, under the direction of the Program Director provides client advocacy, case management, benefit establishment, linkage to Mental Health/Substance Abuse services (if needed), linkage to stable housing and all other supportive services as needed.
A Peer Support Specialist (PSS) is a qualified advocate for the residents. The PSS will provide individualized, ongoing guidance, coaching, support, and encouragement in the time of crisis/need. The PSS will provide training in the use of personal and community resources, such as: SNAP, Medicaid, TANF, Vocational Rehabilitation etc. The PSS will assist in establishing social support networks of relatives, friends and/or significant others.
Upholding Program Standards. Promote the well-being and growth of residents by modeling and encouraging healthy communication, sound decision-making, and upholding house policies. Maintain professionalism with residents, community members, staff and the general public.
Director of Community Outreach
Responsible both internally & externally, for conceptualizing, developing and facilitating the implementation of projects related to community outreach commitments and for being a valued neighbor in those communities that we serve. Requires close coordination and communication with Directors and Managers and to work successfully with representatives from various communities, organizations and neighborhoods with respect to the diverse populations and cultures we serve and seek to serve. Strives to achieve fulfillment of its community relations mission to the greatest extent practicable.
